Plan Information

Open Enrollment is your opportunity to review the medical plan which you have chosen. The Open Enrollment Period will be held for all eligible State, Local Government and Local Education employees from October 1, 2008 through October 31, 2008. Coverage changes made during Open Enrollment will become effective on January 3, 2009 for State employees paid by the State’s Centralized Payroll Unit, and January 1, 2009 for all other State, Local Government and Local Education employees.

To make a change to the medical plan to which you have been assigned, you must submit an Enrollment Application. Please refer to the SHBP/SEHBP website for more information.

Completed enrollment applications must be returned to your employer by October 31, 2008 (or the date established by your employer). Please do not send enrollment applications directly to the SHBP, SEHBP or Horizon BCBSNJ.

About the Plan

Nationwide – NJ DIRECT members have access to an extensive network of physicians, health care professionals and facilities to meet their health care needs. In New Jersey, members access the Horizon BCBSNJ Managed Care Network. NJ DIRECT also provides coverage for members who live or travel outside of New Jersey, throughout the United States and around the world, through BlueCard^® PPO.

Preventive Care – NJ DIRECT provides in-network coverage for most preventive care services such as annual physicals, well baby/child medical care, immunizations, annual vision exams and annual gynecological exams when services are provided by an NJ DIRECT participating physician. Access to preventive services enables our members to stay healthy.
